When a volunteer leaves the organisation we ask them to complete an activity evaluation form. We do this in order to better understand why people leave and, where necessary, make changes that will hopefully improve the way that Lincolnshire County Council engages with volunteers.
You can choose to complete the form on your own or with your manager/supervisor.
Your completed form will be reviewed and used information and statistical purposes.
There could be issues raised in your activity evaluation form that the authority may need to act upon. Such action may include HR raising issues with the line management for further investigation.
